COVID-19 207C/Workers Compensation Claims-Documentation

COBA Membership,Following up on two previous COBA Alerts from April 13th and May 8th concerning 207c/Workers' Compensation & COVD-19.

For all effected COBA members, when you file your claims for COVID-19 207c/Workers Compensation, do not forget the following information in your reports:

1.) All the dates that you were exposed to an individual(s) or environment(s) while in the performance of your duties that you were exposed to and/or possibly exposed to COVID-19. (Include all dates that Exposure Incident Forms were submitted along with copies of those Reports). Since it is difficult to pinpoint a date when the virus was acquired, the best date to use as a Date of Injury would be the date that you started experiencing symptoms, notified the Tour Commander and were subsequently instructed to quarantine. For COBA members that are putting in claims resulting from a Positive Antibody Test, you should use the date that the positive test result was received.
2.) The date you took your COVID-19 test.

3.) The date you received your COVID-19 test RESULT.
4.) The date of you returned to work.
5.) All medical documentation that pertains to your COVID-19 207c/Workers Compensation claim, including but not limited to your COVID-19 Test Result(s). For tests taken at Nassau University Medical Center,  documentation can be obtained by calling Patient Records at (516)-572-6446. Results for Antibody testing that was set up through Nassau County and Northwell are being sent to the email that you provided when you registered.

Keep in mind, on the rare occasion that you do not have a specific direct exposure with a Positive COVID-19 individual, COBA Members have all worked within the confines of the Nassau County Correctional Center, Nassau University Medical Center, Police Headquarters, Courts, and several other locations where numerous Staff, Inmates, and Civilians have tested Positive for COVID-19.  All this work being done while in the performance of your duties as a Correction Officer, Corporal, Sergeant, Lieutenant, Captain, and all Investigator Ranks.
