***Urgent Action Needed*** Re: Open Enrollment Health Benefits

Dear COBA Members;

If you were hired on or after June 1, 2014 and are currently enrolled in the AETNA (OAMC) Health Benefits Plan, you must enroll in a new Health Benefits plan before Open Enrollment closes on December 22, 2021. Effective December 31,2021, the Aetna (OAMC) plan will no longer be an option for Health Benefits through Nassau County. Attached below are the Open Enrollment forms. These forms could also be obtained from Human Resources and must be retuned by December 22, 2021 to avoid a lapse in Heath Coverage.  

Also, The Empire Plan has been sending out new Insurance Benefit Cards to reflect deductible and Out-Of-Pocket Maximums for 2022.  Your ID number and benefits have not changed. If there are any errors with your new benefit cards or need additional cards, requests could be made through Human Resources after January 1, 2022. Your current Benefit Cards are still valid since the ID number has not changed.
