Beneficiary Information Update

COBA Members,

Over the past few weeks Members have contacted the office regarding updating their beneficiary information and checking on their Life Insurance coverage. It is common during times like these for members to think about the protections that are in place for our loved ones.

All COBA Members have $15,000 in Basic Life Insurance for them and their spouse and $5,000 for all applicable children at no cost to the member. Members have the option of $50,000, $100,000 or $200,000 in Supplemental coverage that is paid through payroll deduction.

Attached to this email is the Member Beneficiary form. If you need to update your beneficiary information please fill it out and  mail the original to the office at

Members are reminded to also update their beneficiary information with the New York State Retirement System at https://www.osc.state.ny.us/retire/forms/rs5127.pdf

As a member of the NYS Retirement System you are entitled to a Death Benefit. This benefit is your last retirement statement annual earnings times 3 payable to your beneficiary.

Members that are enrolled in Deferred Compensation can check their beneficiary information at http://www.prudential.com/nassaucounty Create an online account if you have not done so and you can check and update your beneficiary information. You can also contact Jim Cordes at (516) 314-9693 for assistance.

COBA members also have the availability to submit to Human Resources a Termination/Separation Pay Designation Form. This beneficiary designation form is for all monies owed to you from Nassau County (Unused sick, vacation, comp time, lag payroll, etc.) This form will need to be notarized and submit to HR. We recommend that a copy is sent to the COBA office to be added to your file.
